How to Apply for Admission

In order for St. Ignatius Nursing & Rehab Center staff to provide services and care for your loved one, the following forms (link to The Forms below) must be completed and returned to the Admissions Office. This is a state requirement for all potential Residents who may be applying for a nursing home grant. This is usually completed by a family member or the Power of Attorney on behalf of the prospective Resident.

When all forms and documents are received, they will be reviewed to determine if placement is possible. The Admissions Coordinator will contact the responsible party with this decision and to arrange for entrance. 

Completing the Necessary Forms for Admission

The forms needed to complete your application for admission can be printed from this website, mailed to you or retrieved from St. Ignatius in person when you come for a tour. Please send the completed forms via the same format; in person, by email, mail or fax to:

Arranging to Tour St. Ignatius

We welcome you to visit our community, to see our new Rehab Center, the Healing Garden, Solarium and other areas of St. Ignatius. We suggest you call in advance to arrange with the Admissions Coordinator for your personal tour at a mutually convenient time. At this time you can also receive the necessary paperwork to apply for entrance to St. Ignatius.

We accept most major insurance for skilled nursing, therapies, medications, and rehab services.

After your skilled service time has been exhausted, you will become responsible for the cost of care at St. Ignatius.  If the prospective client does not have the financial ability to pay for on-going care, a Nursing Home Grant (Medicaid) would be necessary to pay for the daily cost of care.  The financial information you provide along with the Application for Admission will help us determine eligibility.  We can assist in this process.
